Cardiopulmonary exercise testing (CPET) provides a global assessment of the integrated response to exercise involving the pulmonary, cardiovascular, haematopoietic, neuropsychological, and skeletal muscle systems.
The use of CPET in management of the chronic heart failure patient is increasing in Italy, as in Europe and in US, with the understanding that resting pulmonary and cardiac function testing cannot reliably predict exercise performance and functional capacity and that, furthermore, overall health status and prognosis are predicted better by indices of exercise tolerance than by resting measurements.
The Task Force of the Italian Working Group on Cardiac Rehabilitation Prevention (Gruppo Italiano di Cardiologia Riabilitativa e Prevenzione GICR) endorsed by the Working Group on Cardiac Rehabilitation and Exercise Physiology of the European Society of Cardiology produce regularly the statements which provided recommendations on the interpretation and clinical application of CPET in heart failure, based on contemporary scientific knowledge and technical advances with the focus on clinical indications, issues of standardization, and interpretative strategies for CPET.
The annual Italian Course about Cardiopulmonary Exercise Testing, first took place in 1996 as an “Italian consequence” of ISEIRE (now CPX International Inc) European Practicum, is organized every year in Milan at Centro Cardiologico Monzino by Prof Agostoni. The teachers of Italian Course are Italian Experts in Cardiopulmonary Exercise Testing as Prof PG Agostoni, Prof P Palange, Prof R Belardinelli, Dott. M. Contini, Dott. P Palermo, Dott M Bussotti, Dott A Apostolo, Dott.G Cattadori. In May 2012 the last Edition of the Course was, as usual, an important event for scientific knowledge of clinical indications, issues of standardization, and interpretative strategies for CPET. In Milan at Centro Cardiologico Monzino it is also possible to participate to one week long CPET Practical Stages, organized every month (for informations cardiologicomonzino.it). Moreover one-day courses are organized every year on specific topic where CPET is the major diagnostic tool. This year will be on pulmonary hypertension.
Prof Agostoni is the Chief of Heart Failure Cardiopulmonary Exercise Laboratories and of Heart Failure Unit and the Medical Coordinator of Cardiac Critical Care Area at Centro Cardiologico Monzino in Milan. He is Associate Professor in Cardiology at the University of Milan and Affiliate Associated Professor of Medicine at the Department of Critical Care and Respiratory Diseases of the University of Washington. He is President of ISEIRE from 2005.
Prof Agostoni is the “Italian Representative” for CPX international. The Italian chapter is open to everybody who wish to participate.
